Thrombosis (blood clots) can affect anyone regardless of age, gender or ethnicity.
The anticoagulation clinic team help patients who have been prescribed anticoagulant (blood thinner) drugs, these are given to some patients to prevent blood clots forming inside their body.
One of the anticoagulation medications Warfarin needs regular monitoring.
Once we have the patient's INR (international normalised ratio) result we can advice them of any alterations needed to ensure the INR is kept within the healthy range.
Some patients may be anticoagulated with Heparin injections. They or a carer will have been taught how to administer them or arrangements will have been made for the district nurse to call to administer.
When a patient has been discharged and prescribed an anticoagulant medication the clinician prescribing will give some information regarding the medication until the anticoagulant nurses can speak with the patient to discuss in more detail.
If a patient is on Warfarin then they will attend the outpatient department and have a face to face appointment whilst having their INR blood test taken and then advice regarding any dose changes given.
